About the Role

We are seeking an Assessment Officer to join our People Directorate, delivering frontline services that make a real difference to vulnerable and disabled adults in the community.

As an Assessment Officer, you will carry a caseload under professional supervision, undertaking trusted assessments, support planning, and reviews. You will work collaboratively with health, housing, voluntary, and independent sector organisations to ensure people receive the right support to maximise their independence and wellbeing.

This is a varied role, balancing direct contact with service users, liaison with partner agencies, and ensuring compliance with statutory and council policies.


Key Responsibilities

  • Undertake assessments of vulnerable and disabled people, developing person-centred support plans and reviewing outcomes.

  • Recommend and arrange equipment, adaptations, and rehabilitation techniques to promote safe independence.

  • Carry out joint casework with health, housing, and voluntary sector partners.

  • Identify safeguarding concerns and take appropriate action in line with multi-agency safeguarding procedures.

  • Manage a caseload efficiently, ensuring accurate record-keeping, timely reports, and high standards of service delivery.

  • Contribute to continuous service improvement and the development of effective, value-for-money support services.


About You

We are looking for someone with:

  • Experience as a Social Work Assistant, Occupational Therapy Assistant, or similar role working with vulnerable adults.

  • Knowledge of legislation, policy, and practice in adult social care and disability services.

  • Understanding of rehabilitation, reablement, equipment, and housing adaptations.

  • Strong communication skills, with the ability to work collaboratively across agencies.

  • Confidence in ICT systems and accurate record keeping.

  • NVQ Level 3 (or equivalent) in health/social care or relevant experience.
