Leaving Care Personal Advisor

Job Purpose
We are seeking a dedicated and compassionate Leaving Care Personal Advisor to join the Children's Social Care Service in Sefton. The successful candidate will play a pivotal role in safeguarding and promoting the welfare of young people who are preparing to leave, or have recently left, local authority care.

Key Responsibilities
Provide advice, guidance, and practical support to care-experienced young people.
Take part in the assessment, preparation, monitoring, and review of Pathway Plans, ensuring a person-centred and empowering approach.
Coordinate and facilitate access to appropriate services and opportunities.
Maintain accurate and up-to-date records on ICS and other electronic systems.
Promote positive health, well-being, and confidence in young people, supporting them to reach their full potential.
Identify, assess, and report risks in accordance with safeguarding legislation and local procedures.
Work collaboratively across agencies as part of the wider corporate parenting team.
Attend relevant meetings and represent the department where required.
Contribute to service development through team participation and professional learning.
Requirements
Experience working with young people, ideally within a social care, housing, or youth support setting.
Strong understanding of safeguarding principles and legislation.
Excellent communication and interpersonal skills, with the ability to build trust and rapport.
Competent in maintaining case records and managing caseloads effectively.
Eligible to work in the UK.
