Make a Lasting Difference in the Lives of Children and Families

An exciting opportunity has arisen for a skilled and compassionate Family Practitioner to join a dynamic Prevention & Early Help team, supporting children, young people, and families across the Lewisham area.

This role is central to an innovative transformation programme, focused on early intervention, community connection, and long-term change. The successful candidate will work directly with families to overcome complex challenges, reduce risks, and build on strengths to create sustainable improvements in their lives.

Key Responsibilities:

  • Provide intensive, tailored support to families facing multiple vulnerabilities.

  • Carry out holistic assessments and co-produce action plans addressing issues such as parenting, education, mental health, and domestic abuse.

  • Apply restorative, trauma-informed, and systemic approaches to deliver evidence-based interventions.

  • Ensure the voice of the child and family is central to all decision-making.

  • Work closely with other professionals and services to deliver integrated, multi-agency support.

  • Build trust with families and their extended networks to reduce isolation and increase resilience.

  • Maintain accurate case records in line with GDPR and service policies.

  • Work flexibly, including some evenings and weekends, to meet the needs of families.

Requirements:

  • Minimum 2 years' experience as a Family Support Worker or Family Practitioner supporting families with complex needs (e.g., domestic abuse, mental health, housing instability).

  • Strong understanding of evidence-based interventions, Early Help Assessments, and leading Team Around the Family (TAF) processes.

  • Skilled in mediation, conflict resolution, and direct work with children and families.

  • Demonstrable commitment to trauma-informed and restorative practice.

  • A proactive, solution-focused approach and a genuine passion for supporting families to thrive.
